Mission: To be THE Association for Health Care Delivery System Case Management and Transitions of Care (TOC) Professionals
The ACMA Louisiana Chapter, established in 2017, represents case management and transitions of care professionals who reside or work in the Louisiana region.
Adding membership to the Louisiana Chapter is a great complement to your ACMA national membership because it allows you to better connect with case manager and resources in the state of Louisiana, while still ensuring you have access to the innovative resources and research available through ACMA's network of 8,000 case management and transitions of care professionals.
