ABC Case Management stands for the Advocates for the Betterment of the Community. ABC will advocate for you or your loved one to receive the best level of care. Our Support Coordinators will develop quality person centered care plans, will assist in the development and coordination of community resources, and will monitor your care plan’s implementation to assure your goals are met. We will work with you and all members of your team, connecting you to resources which will enhance you or your loved ones quality of life through several state programs that provide services such as personal care attendant services, vocational, therapy, respite, day programs. and medicaid health services.
